Your gift today to Pandemic Health Workers (PHWs):
• Educates, trains, and helps to deploy compassionate and competent Pandemic Health Workers (PHW) to support the care of individuals in their own homes during the COVID-19 pandemic. These workers include members of the community and health care professionals working in collaboration to care for every neighborhood OSF HealthCare serves.
• Provides specialized training for PHWs, supports the creation of a new digital platform to remotely monitor patient data, purchases and supports mobile health equipment (laptops and tablets), provides personal protective equipment for workers, covers the salary and benefits of additional people hired and provides necessary transportation for PHWs out into the community.
• Provides funding for a new suite of service tools which help to guide PHWs in navigating social and structural determinants of health, such as food security and other factors related to the health and wellness of our communities
PHWs will visit patients virtually and in person, as needed, to check vitals, monitor symptoms and provide care that can be performed outside of the emergency department. They will provide direct counseling and access to behavioral health tools, monitoring, reassurance, mobile health equipment and a means to escalate medical care should symptoms progress.
As medical needs increase, supervision will increase to eventually include advanced practice provider and physician level care. The goal is to keep patients at home when possible and therefore create less pressure on hospitals while also keeping others safe.
