The mission of Project Homeless Connect is to connect San Franciscans experiencing homelessness with the care they need to move forward. PHC makes life-changing social services immediately available to the people who need them most. Our model has been named a national best practice, and since 2004 has spread to over 200 cities in the United States and abroad.
The Problem: There are 6,500 people experiencing homeless in San Francisco right now. Over half sleep outside every night. These individuals suffer from violence, sleep deprivation, and hunger, and face physical and financial barriers to getting the social services they need.
Our Big Idea: Let’s bring these crucial services to the people who need them most, rather than making them spend time and money on navigating a complex delivery system.
The Practice: Project Homeless Connect brings together resources that already exist in the community, like medical care and employment counseling, to create a one-stop-shop event. With a staff of only 13 people, we are able to give wound care, fresh groceries, and public benefits enrollment – among many other services – to over 1,500 people in a single day by harnessing the power of volunteers and community partners.
We know that homelessness doesn't happen Monday through Friday. That’s why our team is available every day of the week. We’re making sure that anyone looking for help in San Francisco has somewhere to turn. By providing care for those who need it most, PHC alleviates the most damaging effects of poverty, and creates opportunity for real change.
